Completing the personal security information

To complete the personal security information:

1. Select your month of birth from the Month of Birth drop-down list.
2. Select your day of birth from the Day of Birth drop-down list.

    

   

    

3. Enter the Last 4 digits of Social Security Number.
4. Click Continue.
