Applying response options to multiple assessment questionnaire items

To apply response options to multiple assessment questionnaire items:

1. Open the assessment questionnaire.
2. Place a check in the box next to the questionnaire item(s) for the response option to be added.

    

   

    

3. Select Apply Scale.
4. Select the applicable scale to be applied to the questionnaire item(s).
5. Scale applied successfully confirmation will display.

    

   

    

6. Click Ok.
7. Click Save.
