Position Information Questionnaire

The Position Information page is where you enter and review information related to the position. Fields within this questionnaire are unlocked by entering a Completion Date in the Verify New Hire Arrived for Duty task. To access this page, click the appropriate new hire Name link from the New Hires tab, click the Questionnaires tab, and click the Questionnaire Name link for Position Information. The following elements are displayed:

Questionnaires

  • Agency/Department. This is the agency or department of the new hire's position.
  • Bureau/Division. This is the bureau or division of the new hire's position.
  • Branch/Organization. This is the branch or organization of the new hire's position.
  • Activity/Unit. This is the activity or unit of the new hire's position.
  • Agency/Department Address Line 1. This is the address of where the position is located.
  • Agency/Department Address Line 2. This is the address of where the position is located.
  • Agency/Department Address Line 3. This is the address of where the position is located.
  • Agency/Department City. This is the city where the position is located.
  • Agency/Department Country. This is the country where the position is located.
  • Agency/Department State. This is the state where the position is located.
  • Agency/Department Postal Code. This is the postal code of where the position is located.
  • Agency/Department Phone Number. This is the phone number where the position is located.
  • Save and Continue. This button saves any changes and takes you to the next page.

The Position page is where you enter and review information specific to the position of the new hire. These fields can be modified. The following elements are displayed.

Questionnaires

  • Position Description Number. This is the position description number of the new hire’s position.
  • Position Title. This is the title of the new hire’s position.
  • Pay Plan. This is the pay plan of the new hire’s position.
  • Series. This is the series of the new hire’s position.
  • Grade. This is the grade of the new hire’s position.
  • Step. This is the step of the new hire's position.
  • Promotion Potential. This is the promotion potential of the new hire's position.
  • Salary . This is the salary of the new hire's position.
  • Salary Type . Select the salary type from the drop-down list.
  • Duty Location Address Line 1. This is the duty location address of the new hire’s position.
  • Duty Location Address Line 2. This is the duty location address of the new hire’s position.
  • Duty Location-Country. This is the duty location country of the new hire’s position.
  • Duty Location-State. This is the duty location state of the new hire’s position.
  • Duty Location-City. This is the duty location city of the new hire’s position.
  • Duty Location-County. This is the duty location county of the new hire’s position.
  • Duty Location-Geo Locator Code. This is the Geo Locator Code of the new hire’s position.
  • Duty Location Postal Code. This is the duty location postal code of the new hire’s position.
  • Type of Appointment. Select the appointment type from the drop-down list.
  • Is this the new hire's initial time limited excepted service appointment?. This radio button indicates if this is the new hire's time limited excepted service appointment.
  • Not to Exceed. This is the not-to-exceed time of the new hire’s position.
  • Appointment Not to Exceed (NTE) Date. This is the not-to-exceed date of the new hire's position.
  • Is this new hire's time-limited excepted service appointment being extended?. This radio button indicates if the new hire's time-limited excepted service appointment is being extended.
  • Work Schedule. Select the work schedule from the drop-down list.
  • Tour of Service End Date. This is the tour of service end date of the new hire’s position.
  • Length of Tour (months). This is the number of months associated with the length of tour for the new hire's position.
  • What DLA overseas activity is the employee being assigned to?. This is the DLA overseas activity assignment for the employee.
  • Does this position include re-employment rights?. This radio button indicates if the position includes re-employment rights.
  • Enter the position title for the position the new hire is being granted re-employment rights to. This is position title to which the new hire is being granted re-employment rights.
  • Position Title. This is the position title.
  • Pay Plan. This is the pay plan.
  • Grade. This is the grade.
  • Please provide a description of the position duties and requirements. This is a description of the position duties and requirements.
  • What time will the new hire's tour of duty start?. This is the start time of the new hire's tour of duty.
  • What time will the new hire's tour of duty end?. This is the end time of the new hire's tour of duty.
  • What days of the week will the new hire be working?. This is the days of the week the new hire will be working.
  • Enter the length of the appointment. This is the length of the appointment.
  • For the purposes of PIV and Pre-Security, what applicant type is this?. Select the applicant type from the drop-down list.
  • Previous. This button returns you to the previous section.
  • Save and Continue. This button saves any changes and takes you to the next page.

Notes:

  • When selecting the duty location for the New Hire:
    • For US locations: the options for county include all counties for the selected state, regardless of the city selected. After selecting a county, the Geo Locator Code options will become available and will be based on the county selected, regardless of the city selected.
    • For foreign locations: the Geo Locator Code options include all options with the same city name regardless of country.
  • When selecting a Geo Locator Code, if you are unable to find a code that includes the selected city and county, please double check the city and county you selected to ensure you selected a valid combination. If changing locations within the questionnaire (country, state, etc), users must select the new values for state, city, and/or county to refresh the Geo Locator Code list.

The Entry on Duty page is where you enter and review information specific to the proposed entry on duty date and date of appointment. The following elements are displayed:

Questionnaires

  • Projected Entrance-On-Duty Date. This drop-down calendar allows you to select the proposed entry on duty date for the new hire. This date may or may not align with the new hire's actual effective date.
  • If the Projected Entrance-On-Duty Date is more than 60 days after the official offer acceptance, select the most appropriate reason for the prolonged delay. This drop-down list allows you select the appropriate reason for delay.
  • Effective Date of Appointment. This drop-down calendar allows you to select the effective date of appointment for the new hire.
  • Was the oath of office administered at the new hire’s duty location?. This radio button indicates whether the oath of office was administered at the duty location.
  • Oath of Office - Country. Select the country from the drop-down list.
  • Oath of Office - State. Select the state from the drop-down list.
  • Oath of Office - City. Enter the city in this field. This field only displays if the oath of office was not administered at the new hire’s duty location.
  • Previous. This button returns you to the previous section.
  • Save & Continue. This button saves any changes and takes you to the next page.

The Payroll page is where you enter and review information specific to the payroll for the new hire. These fields can be modified. The following elements are displayed.

Questionnaires

  • Does your agency require input of the Employer Identification Number (EIN) on tax withholding forms?. This radio-button is where the user indicates if input of the Employer Identification Number (EIN) on tax withholding forms is required.
  • Employer Identification Number. This is the employer identification number for the new hire’s position.
  • Payroll Office Number. This is the payroll office number for the payroll office for the new hire’s position.
  • Payroll Office Contact. This is the payroll office contact for the payroll office for the new hire’s position.
  • Payroll Office Phone Number. This is the phone number for the payroll office for the new hire’s position.
  • Address Line 1. This is the location address of the payroll office.
  • Address Line 2. This is the location address of the payroll office.
  • City. This is the city of the payroll office.
  • Country. This is the country of the payroll office.
  • State. This is the state of the payroll office.
  • Postal Code. This is the postal code of the payroll office.
  • Time and Leave Unit. This is the name of the time and leave unit.
  • State Tax Identification Number. This is the state tax identification number.
  • Claim or Payroll ID Number. This is the claim or payroll ID number.
  • Previous. This button returns you to the previous section.
  • Save & Continue. This button saves any changes and takes you to the next page.

The Conditions page is where you enter and review information specific to the conditions required for the position. These fields can be modified and will vary based on the tasks assigned.

Questionnaires

  • Type of Agreement. This radio button is where you indicate the type of agreement.
  • Non-Appropriated Fund Transfer Agreement Remarks. This is where you enter non-appropriated fund transfer agreement remarks.
  • Enter the minimum number of months, within the specific length of tour, in which the new hire must remain in this position to avoid having to repay transportation expenses. This is where you enter the minimum number of months in which the new hire must remain in this position to avoid having to repay transportation expenses.
  • Please check the box for each functional factor essential to the duties of this position. Select the functional factors essential to the duties of the position from the list.
  • Please check the box for each environmental factor essential to the duties of this position. Select the environmental factors essential to the duties of the position from the list.
  • Purpose of the examination. This radio button is where you indicate the purpose of the examination.
  • Are there any other functional requirements not listed in the checklist?. This radio button is where you indicate if there are any other functional requirements.
  • Specify additional functional requirement(s). This is where you add additional functional requirements.
  • Are there any other environmental factors not listed that the work requires?. This radio button is where you indicate if there are any other environmental factors.
  • Specify the additional environmental factor(s) required by the work. This is where you add additional environmental factors.
  • Previous. This button returns you to the previous section.
  • Save & Continue. This button saves any changes and takes you to the next page.

The Security Clearance page is where you enter and review information specific to the Security Clearance required. These fields can be modified. The following elements are displayed.

Questionnaires

  • Security Clearance Required. Select the required security clearance from the drop-down list.
  • Position Sensitivity and Risk. Select the position sensitivity and risk from the drop-down list.
  • First Name. This is where you enter the first name of the base sponsor.
  • Last Name. This is where you enter the last name of the base sponsor.
  • Phone Number. This is where you enter the phone number of the base sponsor.
  • Previous. This button returns you to the previous section.
  • Save & Continue. This button saves any changes and takes you to the next page.

The Employee Locator Information page is where you enter and review information specific to the employee's location. These fields can be modified.

Questionnaires

  • What is the location code of the employee's current residence?. Select the required security clearance from the drop-down list.
  • Is this a remote job?. This radio-button is where you indicate if the position is a remote job.
  • Is the remote worksite address the same as the new hire's home address?. This radio-button is where you indicate if the remote worksite address is the same as the new hire's home address.
  • Address Line 1. This is the address of the new hire's remote worksite.
  • Address Line 2. This is the address of the new hire's remote worksite.
  • Apartment/Suite. This is the address of the new hire's remote worksite.
  • City. This is the city of the new hire's remote worksite.
  • Country. This is the country of the new hire's remote worksite.
  • State. This is the state of the new hire's remote worksite.
  • Postal Code. This is the postal code of the new hire's remote worksite.
  • Previous. This button returns you to the previous section.
