The Personal Security Information page is where you enter your month, date of birth as well as the last four digits of your social security number so that a unique applicant ID is created for you. This information is used in conjunction with your Biographic information to identify you as an applicant. The following information is displayed:
- Month of Birth. This drop-down list is where you select the month you born.
- Day of Birth. This drop-down list is where you select the day you born.
- Last 4 digits of Social Security Number. This field is where you enter the last four digits of your social security number.
