Personal Security Information

The Personal Security Information page is where you enter your month, date of birth as well as the last four digits of your social security number so that a unique applicant ID is created for you. This information is used in conjunction with your Biographic information to identify you as an applicant. If there is conflicting information-we need to test. Tested as of 12/18/14 and I was able to proceed. Need to revisit this on the next review.The following information is displayed:

• Month of Birth. This drop-down list is where you select the month you were born.
• Day of Birth. This drop-down list is where you select the day you were born.
• Last 4 digits of Social Security Number. This field is where you enter the last four digits of your social security number.
